Next, please follow the steps below to set up your portal and fill out your new patient information. Your information is sent to our office through our secure patient portal. Your information allows more time during our visit and lets Dr. Andrick and the team get to know you better. Please submit the forms a minimum of three business day's prior to your appointment. Before Your Appointment

If you haven't already, please set up your patient portal by following these steps:

  1. Use the link found in the REGISTRATION EMAIL you received on the day you scheduled your appointment.

  2. Or click on the following link and select Request Registration Patient Portal

  3. Then follow the on-screen instructions to set up your account. For a comprehensive guide on completing the patient forms, see this guide.

Once your portal is set up, you'll be able to fill out the required paperwork, view test results, and communicate securely with our team.

Fill out secure patient forms:

  1. Sign into your patient portal Patient Portal

  2. Scroll Down to Getting Started

  3. Click on every link under Demographics.

  4. Click on every link under Health Record.

  5. Click on link Number 1 under Medical History.

  6. Please fill out all sections completely

Day of your appointment

  • Initial consultations are 1 hour and follow up visits are 30 minutes.

  • Plan to check in 15 minutes prior to your appointment.

  • Bring your insurance card and picture ID with you.

  • Bring a copy of any recent lab work.

  • Please call at least 24 hours prior for cancellations or rescheduling.

  • We do not verify benefits or coverage, you can use the insurance worksheet to help you check benefit with your insurance company.
